[GENERAL] Verison 0.1 of model report format

I have made changes in the branch report_formatting that converts the classifier output (expected as key value pairs) to JSON strings and then displays in more human readable format (although jsonify removes linebreaks so the formatting in the flask app is still suboptimal). Let me know how this looks as a V0.1 solution and if I misunderstood any of the requirements for this output formatting. I left the output of the models as dictionaries ({key: value} mappings) to maintain flexibility for now. later we could require, if we want, that key is predicted_class or confidence or score, etc

Can you add the "version" of the report to the report as a field?

Is the Image ID already present?

Does the "test" mean the same thing as "Classifier name"?

I added version as a field for the report. Right now it is hardcoded in app.py, but that could easily change as needed.
Image ID was not present, and I am not 100% sure how we want to define the ID. For now, I am using the filename as the Image ID and also storing the case ID in the report.

Yes, for clarity I am calling that field classifier_name now.
