Subject: [PATCH] docs: Use $remote_addr in nginx config.

The previously used value `$proxy_add_x_forwarded_for` would preserve
any existing value in the `X-Forwarded-For` header and simply append the
true client IP. Using `$remote_addr` instead ensures any existing value
is removed and the header is set to only the true client IP.
