Add working MFA example #48

Enhancement Request
It would be good to add a working 2FA/MFA example for demonstrating with WebInspect/ScanCentral DAST.
Some work had already been started but not completed.
There are three options for MFA:
- Security Code via email
- Security Code via SMS
- TOTP via Authenticator App
The first two require integration to email/SMS provider so more setup required.
The third should be self contained as WebInspect (Login Macro Recorder) already has capability for generating TOTP based on secret (usually from QR Code). Could also be mixed in with Security Question Challenge/Response which again Macro Recorder also has capability for.
