Exec is not refreshed with new added formatters

Issue summary

When you exec something that is later part of the formatter, it's not updated when you call terminal::refresh()

Expected behavior

The exec should be formatter properly

Steps to reproduce

1. Open Repro
2. The second exec is not transformed and highlighted

This is probably a cache issue. Adding a new formatter should invalidate cache.

[jcubic]

The problem is echo_command function that apply formatters with {command: true} before calling echo.

It may require rethinking.
