Health Centre Coverage Screen

This should show a map of the selected location, at the current date, with dots representing all the health and VR facilities in the location.

Active centers (where BR submissions are coming from) should have a different colored dot than inactive ones.

There should also be a summary on this view to show the count and percentage of reporting centers vs the existing centers. This should probably be the only view when this screen is viewed historically.

"Active" centres are centres that have sent in a report for the current reporting period and inactive ones are those that have not.
