Create instructor notes

These workshops are taught by many different people. We need instructor notes that explain:

• The general design of the reader (e.g., note boxes can generally be skipped)
• What are the most important skills for learners to take away
• Which sections to emphasize or de-emphasize
• How to allocate time
• Common points of confusion/struggle for learners (when these can't be fixed by changes to the reader)
