10 failure-mode findings from Cobrust Phase F.3 → Phase I (2026-05-16 to
2026-05-19). Each entry includes Cobrust commit SHAs as ground-truth anchors
and satisfies the second-corroborator requirement.
Slot mapping note: upstream F27-F30 are already in use (persona-testing
and doc-authority sub-forms). Cobrust's local F27-F34 + F35 + F35-sibling
+ P7-stream-watchdog-false-stall map to upstream F31-F40.
Finding summaries:
- F31: ADR batch frame over-scopes without pre-dispatch source verification
- F32: PAIR ceremony unimplementable on single-layer agent platforms (Claude Code)
- F33: Predicate-flip F29 enumeration misses latent consumers (26% miss rate)
- F34: Bidirectional Ref(T)↔T unify produces 142-failure AmbiguousType cascade
- F35: Third corroboration — ADR forward-looking claim ≠ current source state
- F36: Agent skips self-disciplinary rules on first application ("low-risk" rationalization)
- F37: file:NNN anchors drift >100 lines in 14 days on high-churn compiler files
- F38: TEST corpus exit-0 claim invalid on post-merge crate-graph at DEV dispatch time
- F39: DEV commit message preserves original-spec scope after mid-sprint reduction
- F40: 600s stream-watchdog stall signal is false-positive; agent completes post-hoc

… 8 deltas + workflow mode)
The human-facing + core docs had languished at the 2026-05-12/14 '12-day / F1-F30'
era while the methodology grew to F1-F70 + 8 deltas + the 8-dimension audit topology +
the dynamic-Workflow orchestration mode. This refresh brings them current:
- README.md (194→~290): evolution arc + mermaid of the four-batch lineage, F1-F70 /
8-deltas / 8-dim-audit badges, updated layout tree, N=2 case-study status.
- docs/human/{en,zh}/getting-started.md: a worked 'your first ADSD sprint' walkthrough
(sequenceDiagram), the >=4-parallel cap, and 'when to graduate to dynamic-Workflow
orchestration' (Delta 8). zh<->en content-parallel (21 headers each).
- docs/human/{en,zh}/concept-map.md (149→~400): 5-tier topology, 8-dimension audit, the
ADR/finding/snapshot triad, the F1-F70 catalogue, the 8 deltas + Delta 8 workflow mode
— 4 new mermaid diagrams (role topology / dispatch-audit loop / audit topology / deltas).
- SKILL.md: surgical refresh (Parts 1/3/4/5/6 preserved) — Origin/lineage reframed as
the distillation snapshot, new Part 2.5 on dynamic-Workflow orchestration + its
socket-resilience caveat, cross-refs to the cobrust-f31-f39/f41-f43/f44-f70 batches.
- docs/agent/conventions.md: current artifact schemas + lockfile-staging +
chain-generality-verification + honest-signal + Workflow-script authoring conventions.
- reference/EVOLUTION.md (NEW): the F1-F30 -> F31-F40 -> F41-F43 -> F44-F70 + 8-deltas arc.
Provenance: produced by dynamic Workflow #2 (adsd-docs-enrich), which baked in the
Delta-8 socket-resilience lesson from Workflow #1 (each agent retry-wrapped) — all 4
enrich agents returned cleanly this run (no socket deaths). The independent audit stage
returned GO_WITH_FINDINGS, catching one real dogfood violation: enrich agents had
imported uncitable Cobrust *product* stats (~30 commits/day / 9 .cb modules /
RISC-V-under-qemu) that violate ADSD's own §4 benchmark-cite rule. CTO integration
fixed it: removed the uncitable product-stats, kept the citable methodology facts
(F1-F70 counts / 8 deltas / the original 12-day-278-commit distillation window), kept
v0.7.0 only as soft on-disk-tied batch-provenance labels. Independently re-grepped: 0
uncitable product-stat hits; zh/en parity intact.
