When the player dies, an empty glass bottle is dropped.
Observed/Actual behavior
The glass bottle is kept in player hand and still in inv on respawn
Steps/models to reproduce
Give yourself harming potions
Drink them until player dies
Glass bottle still in hand and remains on respawn,
Plugin and Datapack List
No plugins or datapacks. Tested on a clean paper server
Paper version
This server is running Paper version 26.3-143-main@ff3655a (2026-10-02T14:13:32Z) (Implementing API version 26.3.build.143-beta)
You are running the latest version
I can indeed reproduce this on Paper. On a Vanilla server, the bottle gets dropped on death correctly (eventhough on the client it appears as if it was still kept in the inventory at first).
The interesting thing is that upon testing, I actually discovered that whilst you keep the "consumed", empty glass bottle in your inventory after death, you actually also drop a non-consumed bottle of instant harming II as well, effectively a minor duplication glitch.

This in vanilla still has a issue where the result item its added to the same slot but when die, then when respawn the item disapear because its added after the death execute the drop of items... still a strange behaviour.. (i report that https://mojira.dev/MC-312337)

The attached video shows:
Actual result: the empty glass bottle survives death and remains in the player's inventory after respawn.
