Prepare the 0.4.0 OSS release.\n\n- Bump both published packages to 0.4.0\n- Promote the Unreleased changelog entries\n- Prevent the Changesets release-PR action from trying to create duplicate GitHub Releases; the tag-driven release workflow remains authoritative\n- Verification: yarn tsc, yarn lint, yarn build, git diff --check\n\nAfter merge, push tag v0.4.0 to publish both packages.
