The user can now put the 'spell-check-disable' command in the comments to temporarily disable spell checking. The 'spell-check-enable' comment will turn if back on.

Now putting "disable-spell-check" in a comment will temporarily disable
spell checking.  Use "enable-spell-check" in a comment to re-enable it.
Describe the 'spell-check-disable' and 'spell-check-enable'

Also added slash before all single quotes.  This bug was messing
up pandoc's formatting.

The URL test had an obsolete URL for the SimpleITK dictionary.
