MineReflections: make the reflection parse contract classify every record #2336

Problem. skills/Upgrade/Workflows/MineReflections.md:26-36 describes three corpus eras. Long-lived installs also hold hand-appended or older-shape records that fit none of those eras, and the workflow does not say what happens to such a record. In practice, it is silently skipped or mis-read.

Proposed change.

• Make the era table exhaustive, with an explicit discriminator per generation.
• State that a record matching no generation is a hard error that reports its line number and shape.
• Name LIFEOS/TOOLS/Reflect.ts as the only sanctioned writer, so new hand-appends stop creating new shapes.

Why canon benefits. Mining results become complete and auditable on any install that is old enough to have crossed schema changes.
