Convert bank statements from various formats to Open Financial
Exchange (OFX) format.  Rough around the edges, but works.

Supported formats
* Generic CSV
* HTML statement from Samlink online banking service.  Used by
  Säästöpankki (The Finnish Savings Banks Group), Aktia,
  Paikallisosuuspankit (The Finnish Local Cooperative Bank Group) and
  Handelsbanken (in Finland).
* Old (before Danske Bank merger) Sampopankki CSV statement.
