Summary

add cumulative total enrollment N to gsSurv objects
return identical scalar n and N from nSurv
preserve fixed enrollment rates when T = NULL and solve enrollment duration
add ramp-up documentation, vignette, tests, and skill guidance

Validation

full test suite: 3,172 passed, 0 failed, 0 warnings
coverage workflow: passed
