Only the first tool call in each later chunk is currently merged, and interleaved calls don't accumulate correctly. Merge calls by
indexin both the legacy adapter and provider router, covering cases left out by #766.Added 20 regression tests using local HTTP response replay through the normal SDK path. All 220 related offline tests pass with the fix.
