For the layer to be Yocto Project Comaptible it is required that its README contains information on submitting patches. Move the 'pull request' paragraph from CONTRIBUTING.md back to README.md

A backport of #2652 + extraction of the backport process to a separate
file, following the backported commits. This also incorporates a fix
from #2675 .
