Update the runners to macos15 and macos-15-intel, testing against both xcode 16 and 26 (identifying the latest minor version available). The older macos-13 runner is not longer available, and macos-14 will be deprecated soon-ish. Signed-off-by: JP-Ellis <josh@jpellis.me>

I have cleaned up a bit the matrix, and now infer the scheme and destination based on the platform.
